Anshu Singh, MD

Anshu Singh, MD, CMD, HPM, WCC, FPALTC, Associate Professor of Clinical Medicine at SUNY Stony Brook in the Department of Medicine, Division of Internal and General Medicine, is the Medical Director at the Long Island State Veterans Home (LISVH). The LISVH, an academic affiliate at Stony Brook University, is a 350-bed Subacute Rehabilitation and Skilled Nursing Facility. 

A triple board-certified physician in Internal Medicine, Geriatric Medicine and Hospice-Palliative Medicine, Dr Singh is also a Certified Medical Director (CMD) and holds certification in Wound Care (WCC).

Dr Singh has twenty-five years’ experience practicing clinical medicine. Being a geriatrician, she is skilled in taking care of the patient as a whole and not simply treating the diagnosis they carry. When treating her patients, she looks at the whole picture to try to get a better understanding of their beliefs and values which in turn helps understand the patient’s treatment options and prognosis, thereby shaping their present and future health related decision making.

Being a board-certified hospice and palliative care (HPM) physician, along with her extensive experience, knowledge, and compassion, Dr Singh provides medical care to the frailest of the frail patient in the final lap of their journey while also providing comfort to both the patient and their family.

Teaching in her niche professional setting of a subacute rehabilitation and skilled nursing facility has been a passion of hers. She believes that all gerontology trained health care professionals should be vested in training all future health care providers. This is a high priority as there are not enough geriatricians in the country to care for the aging population in various health settings. Being a Site Director, she has the opportunity to educate a variety of trainees; Geriatric Medicine Fellows, Hospice and Palliative Medicine Fellows, Internal Medicine Residents, Medical Students, and Physician Assistant Students.

In her Administrative role as Medical Director, Dr. Singh is involved in various aspects of developing and implementing policy and procedures to improve the care for the special population of geriatric veterans in a State Veterans Home. As part of the LISVH Quality Assurance Performance Improvement (QAPI), which involves interdisciplinary team members and the geriatric medicine fellows, she has been the principal investigator and advisor for numerous Performance Improvement Projects (PIPs).

Dr Singh is actively involved in state and national organizations in her field of practice. On the state level, Dr Singh is a member of the education planning committee, the executive board and holds the position of Vice President in NYS Post-Acute and Long-Term Care Society (NYSPALTC). She represents NYSPALTC on the MOLST/eMOLST Committee for NYS and IPRO-COI. Nationally, Dr Singh is actively involved in various committees within the Post Acute Long Term Care Medical Association (PALTmed). She was recognized as a Fellow of PALTmed (FPALTC) in the Class of 2026, which is noted as the inaugural class of Fellows.  This designation is awarded to individuals who have demonstrated outstanding leadership, commitment, and contributions to the post-acute and long-term care (PALTC) community, and who are active members of PALTmed.
